Information needed for Arrangement Conference

Please bring the following items and information concerning your loved one to the funeral home to make the arrangement conference as comfortable and as beneficial as possible for you:

1. Clothes – Everything but shoes (including underclothing);
2. Military Discharge, with the DD214 information sheet (if applicable);
3. Social Security card;
4. Date and Place of Birth;
5. Parents full names including mother's maiden name;
6. Number of years of formal education;
7. Spouse's date of birth (if spouse is surviving);
8. List of memberships, interests and hobbies;
9. For Ladies – usual cosmetics;
10. Glasses if your loved one wears them most of the time;
11. List of surviving next of kin and their towns of residence including the number of grandchildren and great-grandchildren;
12. Important – a recent photo and either 16 or 28 additional photos spanning your loved one's life, including such photos as childhood, wedding, military service, action photos with family and children (and grandchildren), group photos with family, vacation photos, etc. (3x5, 4x6, 8x10) Choose photos of "the way you want to remember your loved one, the way you want your children and grandchildren to remember your loved one.” Finally, please bring a reference to a favorite scripture, quote or saying that is very special to you or your loved one. We would like to create an “heirloom” for you, to be passed down from generation to generation.
